什么是心肌钙蛋白?
TnC has a molecular weight of 18 KD and is a Ca-binding subunit that binds 2 Ca per molecule. The TnC structures of the myocardium and skeletal muscles are the same.
Since cTnT, cTnI and skeletal muscle heterogenes are encoded by different genes, have different amino acid sequences, and have unique antigenicity, their specificity is significantly better than that of CK-MB isoenzymes. When muscle tissue outside the myocardium is damaged or diseased, CK and CK-MB may increase, while cTnT and cTnI do not exceed their critical values. Since they are extremely low in normal serum, they are significantly increased at the time of AMI, and the fold increase generally exceeds the changes of total CK and CK-MB. Because of the small molecular weight of cTnT and cTnI, free cTn released rapid release of human blood from the cytoplasm of myocytes. The concentration of cTnT and cTnI increased rapidly in blood, which was comparable to or earlier than CK-MB. Although the troponin half-life is very short (cTnT 2 hours, and the half-life of free cTnI reportedly varies from 2h to 5d), its degradation from myofibrils lasts long and can be maintained in the blood. As the time goes by, it has the advantage of an earlier increase in CK-MB and a longer LD1 diagnostic time window. So cTn has gradually replaced the trend of enzymatic indicators. The troponin assay mainly uses double antibody sandwich immunological methods. The detection methods include chemiluminescence and electrochemiluminescence.
5%-8% of cTnT is distributed in the cytoplasm of cardiomyocytes, is a free cytoplasmic protein, and 92%-95% is bound to the thin filaments as a structural protein; 3% of cTnI is distributed in the cytoplasm of myocardial cells. , 97% combined with myocardial structural proteins. When cardiomyocytes are destroyed due to factors such as ischemia and hypoxia, free cTnI and cTnT can be rapidly released from the cell into the blood, and then the bound cTnI and cTnT bound to the myocardial structural protein gradually decompose and slowly release into the circulating blood. Thus, it explains why myocardial infarction is a manifestation of diseases, such as acute myocardial infarction (AMI), where cTnI and cTnT occur earlier in circulating blood and can last longer.
